Bridging the gap between health and social care

Every day, Local Governments face the challenge of ensuring people get the right care, in the right place, at the right time. With recent figures showing 1 in 7 NHS hospital beds were occupied by patients who were medically fit to leave, it's clear how important timely discharges are – both for individuals' wellbeing and for the wider health and care system.

 

Cera works alongside Local Governments and the NHS to bridge the gap between health and social care, helping people return home sooner, easing pressure on hospitals, and making resources go further. Cera’s approach focuses on ensuring we put the right care in place to avoid hospitalisations in the first place. 

 

Keeping those in our care healthy, safe and well in their own homes is part of our mission. Cera’s prevention-led model keeps people out of hospital – with our highly trained, compassionate carers and nurses supported by Cera’s technology to deliver safer, more preventative and effective care.

Accelerating Hospital Discharges
(South West England)

Cera's reablement bridging services support hospital patients to be discharged more quickly and safely, driving better outcomes for the person in need of care and saving valuable money and resources for the Local Authority.

In the South West, patients discharged into Cera's reablement bridging services typically receive six to eight weeks of support. Without this, many would have remained in hospital for at least a week longer – a week that not only delays recovery at home but also comes at a cost of £16,800 per patient.

 

Cera's service costs £3,105 per week, which could result in a potential saving of £13,695 per patient - over £700,000  annually for local health and social care services. More importantly, it helps people regain their independence sooner in a familiar environment.

Trusted Assessor Service
(North West England)

Cera's Trusted Assessor services work hand-in-hand with Local Authorities and hospital discharge teams to make the discharge process smoother and safer. Thanks to the service's support during the assessment process, people can be discharged and return home quickly with the right level of care in place.

Trusted Assessors also assess existing care packages, meaning care plans can be adapted and updated to make sure they reflect each person's changing needs. 100 patients a year are currently assessed in one borough alone, and of these, a quarter saw a reduction in care hours needed each week. This not only improves quality of life, but also creates £100,000 in savings for the Local Authority.
